Preparation for the Test
- Hydrate the day before your test.
- Freeze the included ice pack in the kit at least 24 hours before your scheduled appointment.
- It is required that you fast for 8-12 hours prior to your blood draw.
- You may drink only water, black coffee, or tea without dairy or sugar.
- You must avoid biotin supplementation for 3 days prior to your blood draw.
- You must avoid fish oil for 8 hours prior to your blood draw.
Order Form
You must bring your order form with you the day of your lab draw appointment. You will not be able to complete your appointment without the order form.

Collection Instructions
- Freeze the provided ice packs for 24 hours prior to your blood draw.
- Bring your test order form. The form is included in your kit.
- Bring the following items with you to your blood draw appointment.

The phlebotomist will collect, prepare, and ship your sample back to Boston Heart when they are finished.
Notes: Your kit has been prepaid. You do not need to fill in the payment information on the order form.
